Cannot be combined with other insurance, discount plans, or third-party payment arrangements. Membership is specific to one person and is non-transferable. Unused covered services do not roll over and have no cash value. Services are available only at the participating location(s) listed. The plan automatically renews each year on the renewal date unless canceled in accordance with the membership agreement. You are responsible for payment of any services not covered by the plan.
